Department Digital Forms

Service Description

This service/form is for departments to provide a list of forms from which OIE will create digital forms/service pages in Team Dynamix.

Resolution: 

To request a digital version of your form(s):

1. Download the Excel spreadsheet in the "Attachments" box to the right:
2. Complete Excel spreadsheet.
   • If you have priorities for which forms get digitized first, please indicate them.
   • If you would like your forms organized into categories, please indicate so on the spreadsheet.
3. Click the "Form Request" button to the right.
4. Fill out the form and upload the completed Excel Spreadsheet. 

The initial digitization of forms will take some time. We will contact you for more information about your forms.
